To enroll, complete this registration form and follow the instructions on the form to remit it, along with a copy of your DEA registration and state license to NPR per the instructions on the form. NPR will then establish an account for you and contact you with your login information to their website. From there, returns are simple: just login to the NPR website, enter information about the type and quantity of drugs that you need to send for disposal, and NPR will mail you the required paperwork, including if applicable a DEA Form 222 to document controlled substance transfer. You will then use any packaging and mail carrier of your choosing to send unwanted drugs to NPR for disposal. You will receive a confirmation receipt and invoice from NPR approximately 30 to 45 days after they receive the shipment.
