To enroll, complete this registration form (click here) and follow the instructions to remit it, along with your DEA registration and state license to NPR per the instructions on the form. NPR will then establish an account for you and contact you with your login information. From there, returns are simple. Just log in to the NPR website, enter the information about the type and quantity of drugs that you need to send for disposal, and NPR will mail you the required paperwork, including, if applicable, a DEA Form 222 to document controlled substance transfer. You will then use any packaging and mail carrier of your choosing to send unwanted drugs to NPR for disposal.
